Starting Pay Range - $25.90-$27.07

Position Description:

The Community Care Partnerships Liaison is responsible for developing relationships and agreements with community or regional services, supports, and providers to facilitate care coordination across the spectrum of health services, including access to high‐quality physical health (both acute and chronic) and behavioral health care, as well as social services, housing, educational systems, and employment opportunities as necessary to facilitate wellness and recovery of the whole person.

Essential Job Functions:

  • Initiate formal and informal agreements with community organizations and resources to support CRMHS projects & partnerships. Partnerships may include inpatient psychiatric facilities, residential substance use disorder (SUD) treatment programs, child welfare agencies, peer-operated programs, and others as indicated by client/community needs.
  • Track community agreements and Partnerships in a centralized location
  • Partner with internal departments to identify client needs and seek community resources to fill those needs
  • Develop reports on community engagement activities for CRMHS Leadership
  • Represent the agency at community events and develop presentations for community partners
  • Organize community partners and resources for CRMHS staff to utilize
  • Qualify leads/referrals from Medical and Behavioral health departments as opportunities to engage
  • Track and follow up on successful and unsuccessful referrals and work with departments to reduce barriers to referrals
  • Contact potential partners through cold calls and emails
  • Present CRMHS to potential clients/partners
  • Knowledgeable and incorporate relationship-building opportunities for CRMHS public events
  • Build long-term trusting relationships with clients
  • Proactively seek new business opportunities that include working nights & weekends
  • Outreach to community organizations and resources with a special focus on Latina/o/x
  • Attend monthly and/or quarterly meetings as agency representative and report to appropriate staff follow-up or items of interest
  • Support marketing team in gala and other marketing events

Non-essential Job Functions:

  • Supports agency mission, goals, public events, and objectives.
  • Attend staff and planning meetings as scheduled.
  • Maintain communication and cooperative working relationships both within the agency and community at large, while maintaining a high degree of confidentiality.

Education, Licensing and Experience:

  • Bachelor’s degree in Sales/Marketing or another related field.
  • Current, valid, and active driver’s license.
  • Working knowledge and experience with local community businesses and behavioral Health community is preferred.
  • Diversity, Equity, and Inclusion (DEI) training and experience (preferred)
  • Work weekends/nights as needed
